What is Support Coordination? 

NJ's DDD says ....

 A Support Coordination Agency is an organization qualified by the Division of Developmental Disabilities (DDD) and Medicaid to provide services that assist individuals and families to gain access to needed program and state services, as well as needed social, educational, and other services. 

 

Support Coordinators complete a variety of responsibilities in partnership with the person, his/her family, DDD, and service providers, including: 

• Identifying the person’s support needs and getting to know their preferences 

• Developing the Person Centered Planning Tool (PCPT)

•Developing a Individual Service Plan (ISP)

• Locating options for services that include, based on funds available in the person’s individual budget (Tier Assignment):


- traditional disability providers

-community supports

 -government supports beyond DDD

-natural supports

• Ongoing monitoring of supports and services 

• Responding to emergencies and other service-related needs of the person and/or family
