First, we will schedule a meeting to develop your first Individual Service Plan (ISP) and Personal Center Planning Tool (PCPT), as required by DDD.
The ISP will include information that is “important for you”, to be sure you are safe, healthy, and have assistance for day-to-day tasks.
The PCPT (Person Centered Planning Tool) will be completed with the ISP and focuses on what is “important to you”. The PCPT focuses on your interests, preferences, and dreams.
Planning for now, and into the future. You and your family will choose services that match your goals and dreams.
